Insurance

Our office accepts reimbursement from dozens of insurance plans. We are a preferred provider for many plans as well. Here are just a few of the companies for which we are a preferred provider:

Please keep in mind that there is no charge for an initial examination at our office*, and there is no obligation to pursue any treatment. At the time of the initial exam we are more than happy to advise patients about insurance benefits and assist them in researching expected insurance contributions. If you would like to discuss your specific insurance plan prior to an initial examination, please contact our office manager.

If Dr. Kolodziej is not designated as a preferred orthodontic provider in your particular plan, it is possible that an insurance benefit is still available. In many cases, there is no increase in the “out-of-pocket” cost for a patient who seeks treatment from an orthodontist who is not a “preferred” provider. For example, some insurance companies will pay for 60% of the orthodontic fee up to a maximum allowance of $1,000 if the orthodontist is in their “preferred” network. If the provider is not in the “preferred” network the insurance company might pay 40% of the treatment fee up to a maximum of $1,000. Therefore, in this example, if the treatment fee was $4,000, the patient would still receive the maximum allowance ($1,000) from the insurance company, regardless of whether or not the orthodontist was a “preferred” provider. In other cases, the insurance company may simply reduce the lifetime maximum benefit by a few hundred dollars. Every individual plan is unique.

* unless the exam is for TMJ / jaw-joint pain or for a patient who is in active treatment (braces) elsewhere but is seeking a second opinion
